Transferring IN ?

We are excited to have you join the YouBelong family!

Please read our website to determine if we are a good fit and then give us a call!

You can also request a FREE "Meet-and-Greet" appointment at first, if you'd like to meet us and see the office.

All we need to get started:

Immunization records, and other significant medical records
Copy of your insurance card

Give us a call and our staff will aid you in having your child’s previous records sent to our office and registering your child for their next visit.

We understand that families move, children grow up (21+ years), insurance changes, or sometimes it’s just not the right fit. Our goal is always to provide the very best care for every child, and we welcome any feedback you’d like to share with us.

Please complete and sign a Medical Release Form

After we receive the medical release form, please allow 5-10 business days for the records to be sent.
